How to Select the Best Electronic Medical Record System

With the advent of Electronic Health Records in the late 20th century, healthcare providers and clinics were offered clinical documentation and billing solutions that were designed primarily to support patient care and replace traditional paper-based records. In the 15 to 20 years since digital records became available, advances have been made in the development and deployment of this technology to support a myriad of functions that increase productivity, clinical workflow efficiency, and revenue for healthcare providers.
Most US-based medical providers currently use an Electronic Medical Records (EMR) application, whether in hospitals or clinics. The variety of government incentives that have been offered to physicians have played a key role in increasing in EMR purchase and utilization throughout the country.
When considering EMR technology, whether for the first time or to replace an existing product, a variety of functionalities, potential issues, and desired efficiencies must be effectively and thoroughly evaluated to ensure that solutions meet the need of the individual provider, practice, physician group, hospital or health system. The following is an overview of some of the most crucial aspects to consider when evaluating the purchase and implementation of a new EMR technology.
SECTION 1: CONSIDERATIONS WHEN SELECTING AN EMR PROVIDER
Hosting an EMR? Do you want a cloud with that?
Electronic Medical Records can be accessed and stored in two distinct manners, client-server as well as ‘cloud-based’. There are differing opinions on the benefits of Cloud storage and the convenience of accessing an “off-site” EMR application via a secure internet connection. It is simply the responsibility of the vendor to either provide the solution in the cloud, which is hosted in their datacenter that can be accessed through secure log-in credentials, or a solution in which the specific clinical location will need to contract or create their own datacenter. It is important to understand that there is no difference in functionality between these two hosting methods (client-server, cloud-based) except for those products that may only be offered as a cloud-based application or client server application. In many cases, contracting with a Cloud-based vendor or a vendor that will host the application (potentially a Value-added reseller or VAR partner) will be the least expensive, most cost-effective, and sustainable method of accessing an EMR system.
Billing? Who is going to handle this?
You’ll need to get paid.Revenue Cycle Management (RCM) is the process in which billing/coding and reimbursement for covered services are handled with contracted payers and insurance companies. Most organizations have outsourced this function of their back-office to specific RCM partners that can help the practice maximize their receivables and shrink the total days in accounts receivable. The cost of RCM is usually calculated as a percent of total collections, often making it more cost-effective than employing in-house billing staff. If your organization currently does billing and coding in-house utilizing employed or contracted staff members, it may be worthwhile to evaluate an EMR partner that offers the potential benefit of outsourcing RCM to other, outside agencies.
Are you incentivized to do more outside of the office?EMR applications were initially created to be a clinical data capture system that aided in the process of billing accurately and appropriately for the care provided. These applications have substantially improved in recent years and now include functionalities such as electronic prescribing, advanced reporting, clinical decision support and the ability to create registries that can track and help manage patients suffering from chronic disease.Population health can best be defined as the treatment and management of patients outside of traditional face-to-face clinical moments through the advanced use of support from outside agencies and technologies. Many contemporary EMR systems have expanded their applications to support population health activities. If you are currently engaged in building out population health functionality (care coordination, care management, transitional care) for your providers and patients, there is value in determining if the features they have built into their application in support of population health will suffice. If so, it will eliminate the need to acquire another application that is specifically created for the reporting, documentation and outreach involved in supporting population health activities.
Tell me what happenedWhen evaluating EMR products, it is imperative to consider the data that will be collected and how that can improve patient care and clinic operations. Documentation and underlying clinical data is the very source of patient-truth, it is the foundation of the billing process, it is the calling card for referrals and collaborating practices. How your providers wish to document their clinical notes will be the largest time expenditure for your highest paid resource. Recent studies have shown that for every one minute your providers spend in a face-to-face care moment with their patients, they spend at least three minutes documenting and navigating within their EMR.
- Do your providers want to use a tablet? Do your providers want to continue to use a narrative built by dictation/transcription?
- Do you have providers that want to use speech recognition software?
- Do you have previously built templates that you’d like to leverage for future use?
- Does your clinic have any other specific documentation needs that should be considered?
Having a prepared list of documentation requirements is important for successful vetting of EMR software. Without appropriate methods for capturing the patient story and the clinical moment, a new EMR system may create frustration for providers and fail to improve clinic operations.
Specialty-specifics. No two practices are alike
EMR applications should be fully tailored to the environment, not the other way around. It is an important aspect of identifying the ideal EMR partner that the application work for the use that is common to the current practice. Understand that needs and business lines may change for a practice, but the underlying workflow will most likely stay the same.Your clinic, regardless of the specialization of the providers, has needs for an EMR that can support the nuanced treatments and workflow of your practice. There are EMR systems that tout the ability to work for all specialties and locations, there are EMR systems that are specific to large specialties (think Orthopedics, Neurology, Obstetrics/Gynecology, etc.). Ensure that any product you evaluate has the ability to support your operations from a clinical workflow, documentation and financial perspective.
EMRs can elect to certify their product to show that it adheres to the Meaningful Use Criteria. These criteria are based on the Federal Government’s guidance for EMR functionality to support programs for Medicare patients. Although the functionality requirements that align with Meaningful Use and Certified Electronic Health Record Technology (CEHRT) are important for qualification for those incentive programs, the functionality to support your specific clinical operations is of greater importance than just procuring a product that is certified. Certification ensures the presence of features and functionalities that are related Center for Medicare/Medicaid Services (CMS) program compliance. However, it’s important to note that certification does not test the usability and user experience of the application. Certification does not require that the functionality is user-friendly, intuitive, or will work for a specific workflow. It is important to select a certified EMR for the purposes of qualifying for specific contracts and programs, but a certified system is not a panacea.
Keeping up with the Joneses (Dr. Jones, that is)
It is imperative to understand the technological environment (application and infrastructure) that is present in the network of providers that co-manage your patients and reside in the community (and surrounding communities) in which you serve. The providers that support your patients have clinical data that will enrich and strengthen the patient record of which you are a steward. Creating a holistic and longitudinal record will help to decrease the potential for medical errors, limit gaps in care (or document any gaps that exist), and enrich the providers’ relationship with their patients and their provider colleagues.The best way to connect to disparate clinical data sources is via web-based standards, which are commonly supported through most EMR systems on the market. Be sure that any additional costs to connect to external data sources are well understood during the negotiation of a contract with an EMR vendor. Some examples of important connections / interfaces with external EMRs are as follows: local and area hospitals, nearby freestanding Emergency Rooms and Urgent Care facilities, referring and consulting provider clinics, State immunization and drug databases, Health Information Exchanges (HIEs), other stations of care.
Costs? Are you justifying based on an ROI that is attainable
Looking solely at the cost of the application and/or implementation is not an effective way to evaluate EMR software. While the price tags for EMR systems vary and should be considered, most providers find that the larger financial impact is felt as the EMR impacts clinic operations once it is fully operational. A successful EMR implementation should result in the ability for more patients to be managed by fewer staff. Features that streamline patient outreach, billing, scheduling and data capture also allow providers to see more patients during a traditional day. When weighing the costs of various EMR systems, consider not just the upfront costs to acquire and implement, but also revenue increases and cost savings that will be realized over time.
If you are currently utilizing an EMR and looking to upgrade, make sure your prospective vendors are able to migrate all of your existing data into their application as discrete data elements. You will sacrifice future productivity and efficiency if you do not re-build as many of the active patient charts as possible in your new system. This step can be incredibly tedious and laborious if left to individuals to create, and the process is especially prone to error. There are a variety of potential formats in which to transfer existing data into a new EMR. Full records can be sent using HL7 CDA format, SQL queries, and/or proprietary import functions. If a prospective vendor is unwilling (or unable) to incorporate your historical clinical data into their system as part of your implementation, they may not be the best fit. It is of the utmost importance to prioritize the re-creation of every patient in your new EMR without the loss of any key historical and billing data as current and future reporting requirements highly value the continuity of data.
SECTION 2: STEPS TO SELECTING AN EMR
Step 1: Build a selection committee/team
Buy-in for EMR software must come from all aspects within an organization. Build a team of advisors from all areas of a clinical environment to assist with the evaluation process. Let each resource objectively evaluate the technology and functionality to ensure it matches the needs of the representative group of employees. Consider ways to engage your entire staff in the selection process so they have the opportunity to share their thoughts and suggestions and to ensure their buy-in to the eventual changes to practice operations.
Step 2: Create Potential Vendor List
The EMR space is littered with a myriad of vendors. To best start a list of potential vendors with which to partner, it is important to look at the community around to see what products are in use by complimentary providers. It is also worthwhile to view rankings, recommendations as well as advice published by objective reports and specialty organizations. In many cases, a specialty-specific organization of physicians (primary care, obstetrics, neurology, oncology, orthopedics, for example) will have a list of potential vendors that are in use in other clinical locations. This will help narrow down the potential list from the hundreds to a more manageable 6-12 vendors. Contact each vendor and identify your desire and request to begin the sales process.
Step 3: Demo Day
It is important to have representatives from any vetted EMR company demonstrate the functionality and usability of their application on-site at your clinical location. Let clinical staff use the application in a simulated environment to observe their interaction with the application within the defined workflow of normal clinic operations. This process should be repeated for ancillary staff (billing, coding) should there be an expectation that those resources will also utilize the application.
Step 4: Site Visits
As you narrow your list of prospective vendors, it is important to consider an on-site visit to a local clinic of similar size, specialty and scope that are seasoned users of the application. Make sure that key personnel are available to have frank and open discussions about the application, its functionalities and their use in day-to-day workflows. Understand that every application will have both positives and negatives, as no product is perfect. Ask questions about customizations, upgrades and updates, and most importantly, how support issues are triaged and handled. It may seem like an unnecessary expense to have your staff visit another clinical location to vet a potential vendor, but in the long run, the costs are well worth the effort. Your relationship with an EMR vendor should be as sustainable and transparent as possible. Ask each participant to prepare for the site visit with a checklist of items they want to see in action. Additionally, make sure you facilitate conversation between participants about their needs and goals so you can cover all potential questions that could arise.
Step 5: Contracting
This is often one of the most crucial steps in identifying the best EMR application to procure. Each EMR system’s contracting process will be a little different, as pricing terms can be offered in a variety of ways. Make sure you understand the conditions for term and termination of the contract, and require the inclusion of specific clauses in the contract for non-performance as it pertains to issues relating to service quality and availability of the application. Make sure you have identified data migration points and that the contract language states payment will only be due once the full functionality of the application is released and data continuity from historical systems has been tested and confirmed by your organization. It is imperative in this process to engage legal counsel to ensure contract language Is not overly burdensome and faults of the application are appropriately covered and assurances are binding.
Step 6: Implementation & Troubleshooting
The final step in the process is the most crucial. Even with the best vendor solution selected, a poorly understood and managed implementation can spoil an EMR and limit the on-going value of the product. Leverage best-practices available from your vendor and other organizations to create your project plan and cut-over plan. Create more time in provider schedules to account for the learning curve to the new product and workflow. Announce to patients that the migration is occurring and that certain issues may arise during the transition. During the implementation process, document all concerns and issues identified by staff and make sure each is being resolved before the official go-live of the new EMR system. Test all functionality and interfaces to ensure direct feed of external data is available, and lastly, make sure that a few sample patient visits are tested from check-in to check-out before moving the solution into the clinic full-time.
